When completing the Statement of Error or Erasure of a Name form, please note the following new DMV requirement:

If you are signing the form on behalf of the dealership, you must also ensure that the dealership’s name is stamped or printed in Section 5 of the form.

This is now a required step by the DMV and forms will not be accepted without it.

To help avoid errors, we’ve created an example showing exactly how this should look. Please review and use it as a guide moving forward.

When is a Statement of Error or Erasure of a Name Required?

The Statement of Error or Erasure of a Name should only be used when there is a name error on any of the documents listed in Section 2 of the form.

What About Other Errors?

For example, if you wrote the customer’s ID number incorrectly on the Title Application, you do not need the Statement of Error or Erasure of a Name. Instead:

  1. Draw one single line through the incorrect information.
  2. Write in the correct ID number.
  3. Add your initials and the date of the correction.

Here are the types of errors that do not require a Statement of Error or Erasure of Name form:

  • Customer ID Number 
  • Date of Birth
  • Address
  • Fees
  • Year, Make, Model (On Title Application)

Corrections That Require a Certification Statement on Dealer Letterhead

Some errors cannot be corrected with just a line and initials. The following corrections must be certified on your dealership’s letterhead:

  • Odometer Reading
  • Date of Sale

Please remember, all name corrections must be made directly on the original document, with a Statement of Error attached. Form 502 (Statement of Error or Erasure of a Name) cannot be used alone to make a correction if the change hasn’t been made on the original document itself. As noted in Action Item #4, the correction should be lined out and rewritten on the original document.

The Oregon DMV is adding a new plate to the Special Registration Plate Program. The Oregon Zoo license plates will be available to the public beginning November 3rd 2025. Oregon Zoo plates will be issued to passenger vehicles only and will be available as Custom (Personalized) or Amateur Radio Operator (HAM) plates.

The surcharge for this plate is $40 for both New (4 year) and Used (2 year) registration. The surcharge will also be due at every renewal.

Transactions with $40 Surcharge:

EVR and non-EVR dealers may collect the $40 surcharge instead of a prepaid voucher. These transactions can be processed directly through Vitu or sent directly to the DMV if you are a non-EVR dealer.  

Transactions with a Prepaid Voucher:

If a prepaid voucher is collected in place of the $40 surcharge, the transaction must be submitted to a DMV dealer center (located either in the field or at DMV Headquarters) or mailed to DMV Headquarters. 

Voucher transactions cannot be processed through Vitu.

Within Vitu, Administrators can add, modify, or remove user access. When managing access, it is a best practice to follow the principle of least privilege — grant only the permissions necessary for each user’s job duties, and regularly review access to reduce security risks.

Below is an overview of applications and levels of access.

Applications

  • OR-EVR (Electronic Vehicle Registration) – Grants access to process DMV transactions by sending registration data to the DMV. (Finance access only allows use of the OR Fee Quote tool.)
  • OR-MVR (Motor Vehicle Record) – Pulls the most recent record on file with Oregon DMV, including lienholders and owners on record.
  • NMVTIS – Access to the National Motor Vehicle Title Information System, showing vehicle history information such as title brands, odometer readings, and theft data across states.
  • Theft & Lien – Provides information if a vehicle was previously reported stolen or has any outstanding vehicle lien sales. Does not show Security Interest Holder on record.
  • Shipping – At least one user per location must have Shipping access to activate plate shipments.

Optional Access

  • Interstate – For all 50 states. Vitu Interstate calculates accurate fees and pre-populates forms to simplify out-of-state processes, including optional full-service support.
  • DLDV (Driver License Data Verification) – Verifies out-of-state buyers’ identities quickly and easily. Pulls data from most states via AAMVA.

Levels of access

  • Admin – Full access to all data processed by all users across the location, for EVR allows for registration processing.
  • User – Access limited to data processed by the user only; cannot view reports or data run by others, for EVR allows for registration processing.
  • Finance – Access limited to the finance department for the OR Fee Quote tool only; cannot process registrations.
  • Audit Errors – Essential for all Processing Title Clerks to receive email error notifications:
    • This access can only be granted by your Vitu Account Executive and cannot be granted by the dealership.
    • Always notify your Vitu Account Executive when adding a Processing Title Clerk.

If you prefer, Vitu can assist with adding or removing users from your account. Simply reach out to your dedicated Account Executive, and they will be happy to help.

Removing inactive accounts helps reduce the risk of data breaches

Maintaining active Vitu user accounts for individuals who no longer require access may expose the dealership to significant risks, including:

  • Legal liability
  • Financial loss
  • Reputational damage

It is imperative that access be revoked promptly when no longer required. For assistance, contact your dedicated Account Executive.

Seasonal Relaxation

Let's get spicy this month! Pumpkin spice, that is!

Fall is a time of change, of comfort, of joy, and of peace. Have you ever noticed that fall seems to bring a gentle calm into your life unlike any other month? It is a time where we can take a breath, slow down, enjoy our coffee or tea a little bit more as we look at the changes happening in nature all around us. Fall color is amazing! Oranges, yellows, and reds abound all around us amidst the greens. People seem calmer, walking in the welcome coolness of a nice fall evening. We start to decorate our work and home areas with pumpkins and leaves that reflect the season. Sweaters and hoodies start to come out of storage as the days get a little shorter, a little cooler, and the nights get a little longer. Favorite movies that celebrate the season start popping up on our TVs, inviting us to just curl up with a light blanket and sit, relax, and enjoy them, quietly feeling an inner peace that is so different than the other months of the year.

Yes, October is a very special month. During this wonderful month of YOU, we also think about taking some downtime for ourselves, of taking care of ourselves. It is so important to take care of "us" — if we don't do it, who will?

Remember to still take the time to stretch while at work — get up and walk a bit every hour or so, to keep up your energy levels and to keep that relaxed feeling flowing through you as you power through your day! As the days get shorter and the nights get longer, try and get some morning sunlight in your eyes to help your biological clock (circadian rhythm). Some people benefit from using a special light during the fall and winter months to help them to wake up and sleep better, and help ward off Seasonal Affective Disorder (SAD). It is a real thing, and affects a lot of people during these shorter days and longer nights.

This month’s Treat for YOU 

Now, let's talk about spice! Do you like pumpkin spice? I recently learned how to make a guilt-free, sugar-free, pumpkin spice ice cream, and a pumpkin spice coffee ice cream. I will share the recipe with you, and next month I will share a different recipe with you for another guilt-free item that you can enjoy during the upcoming holidays. 

For the totally guilt-free ice cream, you will need 1 pint of heavy whipping cream, 5 egg yolks, 2 capfuls of real vanilla extract or a vanilla bean, and 6-10 packets of stevia leaf powder or monk fruit. That is our base. It makes for a delicious vanilla ice cream. To make it more fall-like, add a teaspoon or two of Pumpkin Spice, and if you love pumpkin spice lattes, add a tablespoon or two of instant coffee crystals.

Mix it up with a hand mixer, or a whisk, or stand mixer. If you have an ice cream maker, you can then add it to the machine. If you don't have an ice cream maker, do not despair! Just put it in the freezer for 45 minutes, then mix up again for a few minutes, then back in the freezer for 2 hours. Then enjoy this delicious guilt-free treat!
