Effective January 1, 2026 a new multi-purpose form is required for qualifying transfer transactions.

A Duplicate Title application is necessary when a Kentucky title is lost, destroyed, damaged or illegible.

Duplicate Title applications are supported in the Vitu Kentucky solution. A TC96-182 form, which is available in Vitu, is required as supporting documentation uploaded within the transaction. When selected, the form will be pre-filled for your convenience. After selecting, complete all necessary fields prior to scanning.

The TC96-182 uploaded in the transaction can reflect the customer’s physical mailing address, the customer’s mailing address or the dealership's address. 

Note: Using any of these addresses on the TC96-182 will not result in a rejection.

If you request the duplicate title be mailed to your dealership, you must enter the dealership address in the Add Mailing Address field in the portal. When integrated, the mail to address will auto populate in KAVIS.
