




Please ensure that the MV-475 is completed in its entirety. Omitting the dealer representative's name or phone number, for example, delays the correction. Why? Because it requires the Partnering representative processing the correction to review an extensive partner list to find the phone number of the dealer, and hope that the contact on file matches the person submitting the correction. See all required sections/fields highlighted below.


Effective immediately, the below PSBs are revoked. Please do NOT process any work for these businesses. If they attempt to bring work to your office, please make copies of the paperwork if able, then reject back to the PSB, and contact the PSB Unit at 518-473-9947 ASAP. All licensed Private Service Bureaus (PSBs) must complete and submit the PSB10 form with each batch of work that is submitted to DMV for processing.
Although you are able to process the majority of your own registrations transactions through Vitu, there will be some transactions you are unable to process yourself, and therefore these will need to be sent to your local DMV office for processing. In those cases, the PSB10 is required for each batch or individual piece of work you submit.
Note: If you process work for other PSBs through Vitu, you must require the PSB10 form from the PSB. It should remain attached to the PSBs work when sending it to the PSP Unit for review. PSB Partners should retain copies of the PSB10 forms along with all paperwork (per the PSP Agreement) for future audits and or Division of Field Investigation (DFI) requests.
PSBs may download copies of this form by visiting the New York DMV website at https://dmv.ny.gov/forms/psb10.pdf.
In addition, please remember:
- A copy of the PSB employee’s Driver’s License or Non-Driver ID photo document must be attached to all work submitted to Partner/DMV by that PSB employee. The PSB license number, and the valid driver’s license or NDID photo document number, a.k.a. Client ID Number (CID), of the employee submitting the work to DMV must be written in the white margin along the top of each DMV application submitted for processing.
- A PSB employee (runner) MUST hold a valid (unexpired) Driver’s License, Learner’s Permit or Non-Driver Identification (NDID)
- All applications submitted for processing must include a copy of the applicant’s identification document(s).
Note: Ensure that if you are processing work for PSBs, that they have a valid PSB license. If you have any questions as to the validity of their license, please contact the PSB unit at 518-473-9947 to confirm.

Fly E-Bike limited-use motorcycles (LMA, LMB, LMC) failed to meet Federal Motor Vehicle Safety Standards in New York State.
These limited-use motorcycles (LUMs) may no longer be registered or renewed in New York State. All Fly E-Bike LUMs currently registered are no longer valid as of April 30, 2026. Customers with currently registered Fly E-Bikes have been sent a Notice of Registration Termination Letter and a VIN Block has been placed on all vehicle records, which will prevent any future transactions.
The Limited Use Vehicle Database has been updated to remove Fly E-Bikes as an approved make. LUM transactions attempted with a Fly E-Bike (FL\EB) make will return an error message of HR4017: INVALID MAKE.
Since January 7, 2025, New York State (NYS) registered dealer-sold limited-use motorcycles are required to be registered at the point of sale. There may be outstanding dealer-issued Temporary Certificate of Registrations which must be registered since the assigned plate has been used on New York roadways.
Note: Some Fly E-Bike LUMs may have been registered incorrectly as motorcycles (MOT). Issuing offices are reminded to NOT rely on VERIFI pre-populated data. All paperwork and supporting documentation must be reviewed and edited prior to completion of the transaction.
If you are presented with paperwork to register a Fly E-Bike, plates for surrender along with the original ownership document, or have any questions, please contact the Registration Services Unit at 518-626-8911.

Have you received a shipment of plates where the plates did not come as a Have you received a shipment of plates where the plates did not come as a complete set or maybe duplicate plates were received? If so, you must still ACCEPT the order in Vitu as you would normally. For the duplicate set, you may keep the “best looking” set and then destroy the extra set. Be sure to document that you destroyed one set for future audits.
For the plate that did not come as a complete set, mark that set of plates as MISSING in Vitu. Please wait until the day after the order was ACCEPTED to mark the plate set MISSING.
